Planning
Transition
is a coordinated set of activities for a student, designed within
an outcome oriented process, which promotes movement from school
to post-secondary education, vocational training, employment (including
supported employment), continuing and adult education, adult services,
independent living and community participation. The specific coordinated
activities shall be based upon the individual student's needs,
taking into account the student's preferences and interest that
will include instruction. Community experiences that will foster
the development of employment and other post-school, or adult
living objectives when the appropriate. Acquisition of daily living
skills and functional vocational evaluation are provided.
Transition
refers to planning and services that are needed to prepare youth
with disabilities for moving smoothly from school to adult living
in the community after leaving secondary education.
Transition
is not merely an event, a graduation activity, or getting a job.
It addresses the student's needs holistically. It is an ongoing
set of activities to involve the student and his or her family.
The Transition Linkage Coordinator is responsible for managing
the transitional planning with the student.
