CSS Profile Form

  • Only required by certain schools.  Check the form for specific schools before you apply.

This program is conducted by the College Board so you may register on-line at www.collegeboard.com.

Special Conditions

Inform the government and the Financial Aid Administrator or Officer (FAA or FAO) of the colleges to which you have applied of any “special conditions” that affected your family’s income during 2000 or any new circumstances that will affect your income during the next year. These conditions include, but are not limited to:

  • Death or serious illness of a family member.
  • Independent status.
  • Another sibling who is also attending college.
  • Unusual expenses: disabled sibling, loss of work, etc.

Financial aid facts!!!

  • The DEADLINE for the FAFSA is February 1, 2002!  Financial aid funds may be limited for those students who do not apply by February 1st.
  • Filling out the FAFSA form will automatically generate the TAP (Tuition Assistance Program) form (as long as you apply to at least one school in New York State).
  • If you apply for FAFSA electronically be sure to mail in the additional information they require (signatures, etc).
